Health Screening

Screenings for vision, hearing, height, and weight will be conducted for all students in grades K-8. Please keep in mind that these are screenings and if you are notified of a problem, please follow up with the doctor of your choice for further evaluation.
 

Emergency Numbers

If you have moved or had your telephone number changed, please contact the school immediately! If you are working away from home during the day, please see that we have the names and numbers of at least one or two other people who have agreed to take responsibility in your absence, and who can be reached in case of an illness or emergency.
We also advise that you make alternative arrangements for your sick child if you will be unable to stay home with them in case of illness.
 

Medication Policy

Medication, both prescription and nonprescription, will be given under the following conditions:

1.  Prescription medication must be in the original prescription container with the doctor’s name, the student’s name, a current date, name of medication and dosage. Most pharmacies will prepare an extra bottle for school.

2.  A medication sheet (available at the school) signed by the parent must accompany the medication. It must include the time the medication is to be given and any special instructions. If the medication is to be given for fewer than 10 days, a signed note from home will be accepted. The note must include the name of medication, time it is to be given, and how much is to be administered. Medication Permission Form

3.  If your student may need to use an inhaler during the school day, the parent and physician need to complete the permission form for the student to self-administer the inhaler. This form is part of the Medication Permission Form.

Medications given less than four times per day should be administered at home unless ordered differently by the physician. Unless these requirements are met, the medication will be returned to the parent.
 

Immunization Cards

Immunization cards are required by law in order for the student to be admitted to school. Please be sure that all information and signatures are complete. If you have questions, please contact your school nurse or the Hamilton County Public Health Nurses.
 

Illness or Injury at School

Students are to be fever-free and free of vomiting and diarrhea for at least 24 hours before they return to school. This is for your child’s protection, as well as the protection of the other students.
Please call the school each day your child is absent. Some communicable diseases must be reported to the Department of Health and sometimes other parents need to be made aware of contagious diseases. If you know in advance that your child will be absent due to doctor or dental appointments, please notify the school.
 

Excuses for P.E. or Recess

All students are required by law to participate in physical education classes unless there is a medical reason for them to be excluded. In that case, a written excuse from your physician will be required. It is our feeling that all children need to be out for recess for energy release, relaxation, and socialization. We do not go outside in inclement weather. If you feel your child is too sick to participate in recess or physical education, he/she may be too ill to be in school.
