School Registration

Northeast Hamilton conducts school registration through the mail. If you are new to the area, please contact the school to set up a time to come in and tour our school or call the office at 515-325-6234 for any information you may need. Registration packets are mailed out by July 1st each year. There will also be a district-wide newsletter mailing with additional information sent out in August before school begins.

The NEH school office needs the following forms submitted each year for registration (the following forms are mailed out July 1st of each year):
Registration Form (one per family)
Request for School Records (new students only)
Health Update Information Sheet (one per student)
Ala Carte Permission Form
Free & Reduced Application (see below) and Fee Waiver (if applicable)
Student Insurance Information Sheet or Waiver
Ethical Use Policy (one form per student)

Authorization and Permission for Administration of Medication and Authorization for Self-Carry/Administration of Inhaler or Emergency Medication (if applicable)

Dental Screening Form (for all Kindergarten, 9th Grade students, and out-of-state transfer students)

Registration fees per student for the 2011-2012 school year are:

High School (9-12) $50.00
Middle School (6-8) $45.00
Elementary School (KD-5) $40.00


2011-2012 Lunch and Breakfast Charges per day are as follows:

K-5 Lunch Price $2.15
6-12 Lunch Price $2.30
K-12 Reduced Lunch Price $.40
Adult Lunch Price $3.10
K-12./Adult Breakfast $1.35
K-12 Reduced Breakfast $.30
Extra Milk $.30

Students in kindergarten, first, and second grades are served afternoon milk.  The cost of a 20-day ticket is $6.00 and is not included with a free and reduced application.

This form is only included in new student packets. This form is to be completed for all new students and returned to the school.

Health Forms:
Health Update Information - Please complete one for each student.  On the back of this form there is a Fluoride Program sheet for students in grades 1 through 5 to complete and return.  If your student has specific health needs that need to be addressed further, please contact the school nurse at 515-325-6234 or 1-800-435-7104.  Also, if your child is to receive medication at school, please complete the Authorization and Permission for Administration of Medication form.
All Kindergarten, 9th grade students, and out-of-state transfer students must return a completed Dental Screening Form.

Medical Authorization

Free and Reduced Application and Fee Waiver:
Students whose families meet the income guidelines for free and reduced price lunch, the Family Investment Program (FIP), Supplemental Security Income (SSI), transportation assistance under open enrollment, or who are in foster care are eligible to have their student fees waived or partially waived.  Students whose families are experiencing a temporary financial difficulty may be eligible for a temporary waiver of student fees.  Parents or students, who believe they may qualify for any of the above, should complete the Application for Free and Reduced Price School Meals and the Standard Fee Waiver Application and return these forms to the board secretary at the school office.  The Fee Waiver will be included in the mailing, however the Free and Reduced Application will not be available until after August 1st of each year and will therefore need to be picked up at the school.

Insurance Forms and Waivers:
If you are interested in purchasing insurance for your student, you may pick up a brochure at the school office anytime after August 1st.  If you are not interested, please sign the insurance waiver and return to us.

Ethical Use Policy:
When you and your student(s) have read and discussed the NEH Ethical Use Policy, please sign and date the last page and return the last page only to the school.  All students are to sign the form as well as parents.  This permission form will remain in force for future years unless a written request is made by the parent to the technology coordinator requesting that a student not be allowed on the Internet. The Ethical Use Policy was revised this year, so all students need to sign and return this form.

Bus Routes:
For transportation information or questions, please call the transportation department at 515-325-6228.  Bus route schedules will be available in August.

Athletic Physical Form:

Athletic Physical forms are located at the school office and there will be copies at the Jewell and Webster City clinics as well.  These forms must be turned in by the first day of practice.

High School students will typically register for classes before they leave school for the summer.  If, for some reason, your student does not complete their registration for classes, or if you have any other questions about scheduling, please contact Mr. Hocking at the school.


Ala Carte Form:

Middle School and High School students have the opportunity to purchase ala cartes. Students may charge the ala cartes to their family lunch account, if they have their parent's permission. If you wish your child to be able to charge ala cartes to the family lunch account, please complete and return this form.
